Health Insurance Explained What It Covers How It Works and Why It Matters
The Financial Impact of Medical Bills on Families
A medical bill can change a family’s finances faster than almost any other expense. The suddenness and unpredictability of healthcare costs can create a financial shock that is difficult to absorb. One planned surgery like a cataract surgery or LASIK, for instance, can often lead to a cumulative expense that exceeds the financial reserves most families maintain for health expenses.

It is wonderful that you have planned ahead for your medical expenses by securing a health insurance policy. Most active health policies cover daycare microsurgeries—like cataract surgery—once you have completed your policy's standard waiting period for eye care. For refractive procedures like LASIK, IRDA guidelines specify that insurance coverage generally applies if your refractive error is -7.00 diopters or higher.
When planning an eye treatment, navigating insurance paperwork should be the least of your concerns. By making the claims process clear and easy to understand, we want your entire experience to be smooth, simple, and completely transparent. In this video, I will guide you step-by-step through the insurance process so you can make informed decisions with total confidence.
Medical insurance claims are primarily handled through two routes: Cashless and Reimbursement, and each offers unique advantages depending on your priorities.

A Cashless claim gives you peace of mind and convenience. Its greatest benefit is freeing you from the stress of arranging money for your hospitalisation on short notice, and then downloading and filling out insurance claim forms, or directly answering technical insurer queries. At an empaneled clinic, our insurance desk handles all document submissions and TPA coordination directly, allowing you to focus entirely on your medical recovery. But please note a few issues. Most Eye Hospitals work with package rates and certain discounts would have been imposed by the Insurance company..

When navigating health insurance in India, you will hear about GIPSA—the General Insurance Public Sector Association—and its Preferred Provider Network. Formed by four major public sector insurers, GIPSA was designed to standardize costs across hospitals by creating fixed, pre-agreed package tariffs for common surgical procedures. This was introduced sometime between 2008 and 2010.
On paper, this framework offers a few clear advantages. It protects policyholders from arbitrary price inflation, streamlines cashless pre-authorizations, and leverages massive public-sector volume to keep overall policy premiums relatively competitive.
However, the primary drawback of the GIPSA model is its rigid, one-size-fits-all capping system. Because procedures are locked into fixed package rates, there is very little room to accommodate advanced medical technology, specialized surgical techniques, or premium custom implants. To avoid unexpected surprises, it is important to understand how these package limits interact with modern eye care. While standard cashless approvals cover foundational treatment under the pre-agreed tariff, advanced options like customized lenses or specialized disposables fall outside the fixed package cap. As a hospital, we ensure complete transparency: the core package amount is settled directly by your insurer, while any voluntary technology upgrades opted by you for your procedure are billed as out-of-pocket expenses that you are needed to be settle directly with hospital. These are not-reimbursable by your Insurance company under your cashless policy.

On the other hand, by opting out of Cashless, Reimbursement claims offer distinct financial and operational flexibility. Because reimbursement isn't strictly tied to rigid network hospital package rates or pre-authorization sub-limits, it often results in a reduction in strict policy cappings and premium options. Furthermore, in many complex or non-standard cases, filing a reimbursement claim with comprehensive itemized bills can actually lead to better overall settlement numbers. Reimbursement also gives you the absolute freedom to seek treatment at any registered medical facility of your choice, regardless of whether they have a formal tie-up with your insurer or not.
At Sukruthi Eye Care Microsurgical Centre, we work closely with you to guide you through both cashless pre-authorizations and detailed reimbursement paperwork, ensuring your claims process is completely transparent from start to finish. It is your choice and privilege to select either Cashless or Reimbursement method of Health Insurance Claims.
At Sukruthi Eye Care Microsurgical Centre, we aim to make your cashless approval process as smooth and transparent as possible. Here is how our workflow operates step-by-step.

First is Document Verification and Pre-Authorization. Much before the day of your planned procedure, we will need to you to please bring your TPA health card or policy copy, government ID proof such as your Aadhaar card and PAN card of the patient and the proposer- both need to be appropriately self attested and your consultation report to our insurance desk. Our staff will complete the cashless pre-authorization form with the doctor's advised treatment details and estimated costs, then submit it electronically to your insurance provider.
Second is Initial Approval. Most insurance TPAs take between two to four hours to review and process pre-authorization requests. Once approved, the insurer issues an initial sanction letter specifying the approved amount, eligible room category, and any policy deductibles. We will immediately notify you about this. Most Insurance Companies also notify you of the same directly too.

Third is Admission and Treatment. On the day of your surgery, you can proceed comfortably with your treatment without needing a major cash deposit for the sanctioned amount. Any non-payable items including the costs over the initial approval for the services or material either opted by you or necessitated during the course of your treatment are explained. Payments to these are to be done at our counter against the bills raised.
Finally, Bill Clearance and Discharge. Following your procedure, our billing desk compiles your complete itemized bill and discharge summary, transmitting them to your TPA for final sanction. The TPA reviews the final summary and issues a final approval letter—typically taking one to three hours. Once received, you simply sign the official claim form, pick up your folder and post-operative instructions and medications, and return home to focus on a comfortable recovery.

Once your insurance company settles our claim against their final approval, any excess amount paid by you will be refunded back to your bank account at the earliest. The Insurance company will also intimate to you when they have settled the Hospital amount. In case there is a further shortfall in the settlement, we shall intimate you to please pay the balance amount. However, it is very rare that this happens.

Opting for Reimbursement makes the discharge process simple and efficient. Once your full bill is settled as per your selected tariff, we immediately provide your complete payment receipt, a detailed discharge summary, an itemized billing breakdown, all diagnostic reports—including Ultrasound A-Scan Biometry—and a completed Claim Form B.
To submit your claim, download the primary claim forms from your insurance portal, complete your details, and attach our paperwork along with your ID copies, a canceled cheque, and any requested policy documents. Some companies accept online uploads, while others require physical courier submission or both. Always remember to make a photocopy of the entire set for your records before submitting.
